Congratulations on qualifying for the 2009 MCYSA/USSSA Summer National Invitational Championships. Those of us that work on this tournament are proud of the event and will do whatever we can to keep the focus of your time on the tournament and its related events.
With that goal in mind, I beg you to preregister your team 7 days prior to arriving at the tournament. I have yet to meet a coach, player, or parent that would choose to be inside registering rather than preparing to play ball.

What's involved in preregistering?
Prior to arriving at the tournament your team contact person will need to complete the following and send all the materials to me as registration coordinator. Please note that you'll have to download two forms and then make copies for each player. It might be wise to delegate this responsibility to a person proficient with a computer. Also, it is wise to schedule a team meeting to address the MCYSA/USSSA registration process with players and parents so that accurate information and required signatures can be obtained. In an emergency situation, this information could be priceless.

THE REQUIRED FORMS
Program Waiver And Release Of All Claims Form - Form 1
Form 1 must be signed by both the player and a legal guardian/parent.
Consent By Parent/Guardian For Medical Treatment Of A Minor - Form 2
This form must be completed for each player and signed by a parent/guardian.
Collect Birth Certificates
A photocopy of the birth certificate for each player on your roster must be given
to the Tournament Committee as part of registration. These copies will NOT
be returned to you.
Proof of Insurance
Your team must provide the Tournament Committee a certificate proving that
you have accident, medical, and liability coverage for your team and your players.

THE CUBS/ASTROS GAME OUTING- Tuesday July 28
For $ 55 or $85 you'll receive a round-trip motor coach ride and a ticket to the
game at Wrigley Field on Tuesday July 28. Ticket orders will be filled on a first
come, first served basis. The $85 seats are in the bleachers and the $55 are in
the reserved portion of the upper deck.
MCYSA/USSSA NATIONAL INVITATIONAL CHAMPIONSHIPS SOUVENIR SHIRTS
This pre-order only t-shirt will have the tournament logo imprinted on the front with all tournament teams listed on the back. Regular price is $ 25 but pre-order pricing is $18.50 for all sizes.
MCYSA/USSSA TOURNAMENT ADMISSION PRICES
Passes ordered and paid for in advance can be picked-up at registration.
Once your team has completed forms 1, 2, and 3, the forms, payment for your orders, copies of each players birth certificates, and your teams proof of insurance must be sent to:

THIS PACKAGE OF REGISTRATION MATERIALS MUST BE IN RECEIVED
NO LATER THAN TUESDAY JULY 21ST. If you're in doubt about meeting this deadline, please overnight the materials. Remember to mark, "Signature Not Required" on the mailing form.
What does your team contact person do on Registration Day (July 28)?
If your registration materials need to be adjusted, remember to bring all needed documentation to the Crystal Lake Holiday Inn (800 South Route 31 and Three Oaks Road telephone (815) 477-7000) between the hours of 9 am and noon. The Holiday Inn is 1/2 mile south of Route 14 on Route 31.
All teams must register in person before they can play in any games. If there are any teams arriving late, those teams MUST register in person with the Tournament Director for your age division prior to playing. If you'll be registering late, please contact Jack Sebesta at (815) 482-1565.
Hat Pin Exchanges
A very popular part of our experience is the exchanging of hat pins between players
and their supporters. It's recommended that you bring a pin that is unique to your
state or country to exchange with other players during the tournament.
